A sworn certificate signed by a member of the commission in which the following points shall be specified: i. That the permitted has been given an opportunity to appear before the Commission as required pursuant to rule 3-13 of the rules or order issued pursuant to this rule. ii. That the permitted has fulfilled this opportunity fully. iii. That all necessary information has been furnished to the Commission in full, including each person's name, address, telephone number, fax number, EIN or employer identification number, etc. b. A completed application must have a signature to be taken by any person.
